2017-18 ID Badge Renewal Procedure

Reminder: Local 500 ID Badges are mandatory.
You can renew your Local 500 ID Badge using one of the following methods:

Download, print and complete all forms and mail them along with a self addressed stamped envelope, or
contact Local 500's office at (954) 202-2624 to schedule an appointment.

In order to save yourself some time, you may download the necessary forms in advance and bring the completed forms with you on your appointment date.

Please note that if any worker who does not have a current Local 500 ID badge will not be called for work.

* Effective December 31, 2016, Avitus Group (Theatrical Payroll Service of Florida) will no longer be printing checks and will require that all employees sign up with either Direct Deposit or a Pay Card.

ALL WORKERS MUST fill out the Avitus Group Form and choose either direct deposit or pay card. You MUST provide a voided check for direct deposit. It is provided in the download  for the ID Renewal  2016-2017
